Director of Social Services Duties
- Plan, organize, and direct all functions of the Social Services Department
- Complete and oversee social service assessments, care planning, and documentation
- Support residents and families with discharge planning, care transitions, and access to financial, legal, or community resources
- Facilitate grievance resolution and advocate for residents’ rights and dignity
- Participate in interdisciplinary care team meetings and QAPI initiatives
- Oversee advance care planning and end-of-life discussions in accordance with resident wishes
- Supervise and mentor social work staff, ensuring compliance with federal and state regulations
- Participate in resident and family councils, community outreach, and facility committees
- Uphold confidentiality, ethical practice, and professional standards of care
Requirements
- Bachelor’s degree in Social Work or a related human services field (sociology, psychology, gerontology, etc.)
- At least one year of supervised social work experience in a healthcare setting
- Knowledge of state and federal long-term care regulations
- Strong leadership, communication, and problem-solving skills
- Compassionate, organized, and team-oriented approach
